Judder in Reverse & slips out of 2nd

I have an 89 Excel with a four speed manual transmission. I have made all the pedal and linkage adjustments for the clutch. Yet, it still judders when going in reverse and pops out of second gear. It doesn't slip in any of the forward gears. What else can be done to fix this problem? Is it clutch time? Thank you very much.
