Difficult warm starting - any ideas??

OK, I'm sure this has happened to others, but I haven't been able to find a clearcut answer, so I thought I'd let everyone give their opinion. Included is as much detail as I can remember, so I'm sure this will be a long one! Thanks for any ideas!

My 2000 Impala 3.4L (currently 112,000 miles) has had a hard time starting when the engine is warm, never when cold. It used to start and the idle would fluctuate up and down a couple times then level off, but lately it fires and immediately dies. If I give it a bit of throttle, it will start fine, problem is it usually runs the RPM up a bit, which I know isn't great at startup, plus nobody else can start it (which I suppose isn't all bad, keeps theifs away!) I would say this started at about 70,000 miles, and has progressively gotten worse since then. I purchased this car brand new, and its never been in an accident, also, all the regular maintenance items have been done pretty much on schedule. The only other major fix that might affect this include the intake manifold gasket at 58,000 miles (was sucking a quart of oil every 1,000 miles!) Plugs, wires, coil packs, fuel filter, air filter all recently changed, had the fuel injectors & TB cleaned, even got the meter out and tested the voltage on a couple sensors per the repair guide I have, all seem to be within specs. Oh, the fuel economy seems to be down a bit from what it used to be (probably 10-15% lower on average) This has been a gradual decrease which started about the same time as the hard starts.

So if anyone has any ideas, let me know! I'm not opposed to changing a few parts as long as they aren't the most expensive ones first!

Should also mention there are no codes being set. I've had this checked several times. only once was their a code for a misfire, which was my fault for not getting a plug wire on tight!
