attachments not 'attaching?'

is anyone else having trouble with attaching pictures and diagrams? I've had this happen a few times. Click the attachment button, upload the photo, then attach it to the posting. It appears to attach and then when you post the picture isn't there at all.. just blank.

any ideas?
