Rought idle at start sometimes

Mechanic wants to replace Idle Air control valve...sometimes spraying throttle body cleaner into the ilde air control valve with high RPM helps.

any thing else i should consider than changing part?

well, I'd do a search on IAC cleaning as it's a very "discussed" procedure and is quite simple to do. I'd buy some spray cleaner specifically designed for IAC cleaning and follow the instructions. I've found with careful cleaning the IAC works great. If you're at all mechanical, tackle the job yourself and thoroughly clean the IAC. I'd bet you'll do a better job than the mechanic. I'd certainly be doing this before replacing the IAC. Also, your air filter is new or almost new right?

yes just put in new air filter and on top of it all the time. Mechanic also said to put premium gas now and then might try that.

if it's not necessarily an IAC problem, the other thing you should consider is the age of your fuel filter. They're cheap to buy and should be replaced annually I'd suggest. If the idle is still only at start though, I'd be leaning more towards IAC problems still tho.

yes it is only at start so i will look up and try to clean and i think fuel filter is about 3 years old might be replacing that next time i do some work on the van.

Agreed. It could be vacuum leak at intake manifold seal or gasket, at IMRC...
Disconnect MAF sensor before cold start and see if symptom goes away. If it goes away, the problem is really vacuum leak.
