Strange Problem with 60%+ Throttle

The video shows the problem I have been having.

I'm not sure what has been causing this I believe it to be the transmission slipping or maybe it's just low on fluid. Any time you "get on" the gas or just accelerate on the highway it seems to slip and stop for a minute and drop the RPMs.

Does anyone know what causes this or could be a solution? I know my ABS and brake lights are on but those have been on before and this has never been a problem. The engine has been swapped and the problem started to happen after that happened. 180,000 miles and has been treated with care, the newer engine only has 95,000.

Looks like the check engine lite is on, get the codes read and post back.
