06 Grand Caravan Disc Rotor and Pad replacement
How hard is this to accomplish - any special tools required ?

shouldn't be much different than the 05, and once you have the wheel off, and the calipers out of the way, the rotors should come right off. You might need to tap it with a rubber mallet to break them free, but there's no special tools or hardware that you need...
to install them, you just pop 'em in place, and put the calipers back on. I can HIGHLY recommend the ceramic pads. Less dust, and with those mag wheels, they can get ugly quickly...
