renault clio cambelt failure

My 2005 renault Clio has suffered 3 failures of retainer plate bottom cambelt pulley. Now the cambelt has broken due to excessive wear belt tensioner. My question is should the mechanic not have checked or replaced the tensioner on the previous repairs?
