Present this printed e-certificate and personal identification.
Test-drive the all-new 2011 Kia Optima.
After you take your test-drive, have the sales representative complete the
required dealer information.

April 15, 2011, to receive your $25 Visa
prepaid card.
Approximately 6 to 8 weeks after mailing your completed e-certificate, you'll receive a $25 Visa prepaid card by return mail.

Please fill out this e-certificate completely. Any missing information will cause a delay in processing.

PREPAID CARD DISCLOSURE: The Visa prepaid card is not redeemable for cash or usable at any ATM. The plastic Visa prepaid card itself expires 12 months after issuance; provided, however, the funds available on the prepaid card do not expire. If funds remain on the card after expiration of the prepaid card, please contact Customer Service at 1-877-883-4913 to request the issuance of a new card. Applicable replacement card fee of $6.95 will be deducted from the current balance of the prepaid card in order to issue a new prepaid card. Terms and Conditions apply to the card. Subject to applicable law, a monthly maintenance fee of $3 (USD) applies, but is waived for the first 6 months after the card is issued. Your card is issued by MetaBank™ pursuant to a license from Visa U.S.A. Inc. Cards can be used at any merchants that accept Visa debit cards. Kia reserves the right to substitute a check of equal value in lieu of a Visa prepaid card at its discretion.
