That is where you need a good engine capable scanner.

Check history frames and get the fail frame of when it set the codes.
Not sure but if your fuel system and pressure checks out you may have some dirty injectors.
Again a engine capable scanner will run a injector balance test.

Western Washington has seen a lot of crappy gas at the "discount" stations. Doing the injector cleaning and filling up with some name brand gas has been solving the exact condition you're complaining about. You don't state anywhere what fuel you use, but thats my guess.

I get my gas at shell and I use plus and V power (super) every fourth tank, I get it from a gas station that is on a major highway in Jersey....
