Warped Flywheel?

I just a new Exedy stage 1 clutch in my Miata and after 3 weeks it's already slipping in every gear.

My only assumption is a warped flywheel.

If anyone knows how this would happen please let me know.

Did you have the flywheel machined when you replaced the clutch? Did you allow a proper 500 mile stop and go break-in before you started hammering on it?
