03 suburban heater fan wont shut off

I have a 2003 suburban. The heater/AC fan will not shut off or change speed. When I shut the truck off the fan still runs. I ended up pulling the fuse to shut it off, but when I replace the fuse it comes back on.

Any suggestions would be appreciated thanks
