Loose headlight assembly - 1992 to 1995 Caravans/voyagers

I have a loose headlight assembly (passenger side) on my 1995 caravan and car will not pass state inspection. Don't know if it is the assembly or the headlight housing. How does one get access to the assembly or headlight housing? I have removed every exposed screw I find and have driven around town for the last week, but still intact.
