Mercedes Benz Vaneo Misfire

I own a Merc Benz Vaneo 1600 petrol 03, I have done a diagnostic check which shows code P0204 Injector Circuit Open Cylinder 4, what does this mean and could it be a faulty coil Pack? I have now found that petrol is coming out of the exhaust?
