2000 3.5L AIR pump system

Despite patching a small hole in the plastic outlet air pump hose, I continue to get code P0410 and a check engine light. After resetting the code and CEL, the CEL takes a long time to reappear (months), but the OBDII scanner shows the secondary air pump sensor to be incomplete/not ready. All other sensors show as 'ready'.

With inspection coming up, I need to fix this. The pump seems to be running find (no screeching, runs for ~30sec at startup, is pumping air). Since it seems that the pump and relay are fine, I suspect that replacing the outlet hose(s) and control valves between the pump and manifold will fix this. Can anyone provide the part numbers for the outlet hoses and these valves?
