Overheating

I have a 1992 Pontiac Grand Am SE. When we are on the highway its fine stays close to 100. When we do town driving it gets close to 220. When we pull off and check the engine the engine is cool, but the upper radiator hose is hotter than ever. Any suggestions would be great.
