besides, AF's search is still very hit and miss when it comes to it pulling up threads or not.
one of the tidiest I've seen is Alex Kustov's as seen
here except it isn't 100% accurate to the real but the important thing there is how he made his hinges.
the difficult bit is how to get the door to swing into the body like a real car does instead of swinging out as a die cast tends to.

here but not that clearly i'm afraid.
essentially, the hinge is a styrene tube cut into three sections with the top and bottom section fixed to the door and the centre section attached to the large flange piece you can see. A metal rod is used inside the styrene tube (sections) to complete the hinge and the flange is fixed to the body.
As for filling in the missing door jamb detail, that would depend entirely on what car/kit you're building and what materials you're used to using to doing body mods.
