Have you ever changed fuel pump????? Check fuel pressure with a gauge after fuel filter, if you can tape it too windshield wiper and observe from cold to warm up to see if you are losing fuel pressure.

ANother thing to check is the coil, you could try using a hair dryer and heated up the coil when the engine is completely cold and see if it acts up then.

ANother thing is remove distributor cap and rotor, then plate in distributor to see if there is any dust on the optical sensor (camshaft sensor), if so the bearing on shaft is going south and will need to replace distributor, the optical sensor could also be faulty.
