If you bought a reman MAF sensor from A-1 Cardone, it's very possible that it could be bad again as that brand is questionable at best.

From your symptoms, I would have the fuel pressure checked, as it sounds like you're running very lean which caused the cat to burn up (if was even bad) and the hole in the intake manifold. When you had the upper plenum fixed, did they also change the lower intake gaskets? They are a fairly common issue on these cars and could be another cause of your symptoms, especially if you're mysteriously losing coolant. Bad intake gaskets can cause an internal coolant leak which can get in the converter substrate and ruin a new converter, and also a vacuum leak which will cause driveability issues.
