Haven't been into hondas for a while so I'm not 100% sure on pricing, but the last time I got estimates on an auto to manual swap it was about $1200, but that was for a 93 Integra which was going to have a B16 tranny with a cable clutch. It will be a little more for a car that will need a hydraulic clutch setup like yours.
You could get new gauges if you like. Since you have an LX it has a tach already right? Getting a new cluster is totally up to you.
Your insurance won't increase. It might if you tell them that you did the swap, but there's really no point in reporting mods unless you have some expensive mods or are extensively modified.
No paperwork to be done, just make sure you keep all paperwork from the shop that does your swap.
Lastly, your intake has nothing to do with the tranny. Don't worry about it.
