I'm pretty sure the CAI from HS is 3".

As for where to place the valve, anywhere before the MAF is fine - the higher in the engine bay the better.

The real issue comes up when driving through a puddle which is about as deep as the lower edge of your bumper. At that point the water has enough wieght to it (or around it) to physically push itself into your filter and then get vacuumed up.
