It appears that you have a bad connector that may be causing your problem. Although the problem may be the fan itself. The fan has two speeds and it should come on at high speed when the AC is on. Try replacing the fan relay if that doesn't work you may need to replace the fan assembly along with the relay. The temp sensor could be bad too.

Would the relay cause the fan to work or not when the connector on the fan motor has to be jiggled, with some force, to make it come on?
