At this point my money is on an internal mechanical problem. If its due to low oil pressure and/or excessive valve clearances, try heavier (much heavier) oil, such as 20w-50.
Yes, I know its not ideal for cold weather start up, but at this point you have nothing to lose. Likley you will need an engine rebuild so heavier oil may help you squeeze a few more miles out of it.
However, many mechanical problems, like a spun bearing, piston slap, loose cam chain etc will not be improved by heavier oil.
