Your explanation/description should be a model for questions asked in this forum in general, any way you have all but eliminated everything but the module. problem is with it's seemingly partial operation it is hard to be sure. Check the grounds to the engine and chassis, make sure the connector to the module is clean and tight, look for any frayed wires or connector pulled out in that icm harness. An erratic/ weak signal from the crank sensor has been known to cause this, but without a scope it is impossible to check the 3x vs 18x signal.
