Never had the rear rotors off, but it should be the same as the front. Take the caliper off and pull the rotor off the wheel studs. It might be rusted on like you said. I'd take a dead-blow hammer and tap on the back side of the rotor and see if it breaks loose. It shouldn't require much and with a nylon dead-blow hammer, you won't cause any damage.
