Add some more. It's the release of refrigerant from the can that causes the needle to "go crazy". Eventually your lowest reading will come up. Take the gauge measurement when you're not releasing refrigerant.
I would only start troubleshooting if the second can of 134a doesn't do the trick.

Do you have the type of 134a with the pressure gauge and refrigerant all-in-one?
