Well it's possible that you used too much grease when you installed the clutch, usually causes it to start slipping around 5000 miles after install. With hard launches it's also possible that a spring broke in the clutch or something on the pressure plate. Sounds clutch related to me though, as soon as the engine puts some torque to the clutch it slips. Can you get it to grip above 2k at all? I doubt it's an adjustment issue but it is possible...that's easy to check though so i'd check that first.
