yes they are hooked up correctly all the plugs are keyed except for the power (+ and -). So it can't go any other way. I was using a 12v power supply it was putting out 11.9 - 12.1v under tests.

My dad said it's possible it could be normal depending on how the transformer was designed so I'm not sure. It's inclosed in a case. I think I'll just leave it in a really accessable place when I install it incase it should die again and maybe pick up another transformer while I'm at it.
