Well it surely was NOT a "loose" injector, if the injector hold-down bolts are even the slightest bit loose, they WILL leak oil into the fuel, and you will need new injector o-rings to fix it.

Yes, a clogged cat will do this.

Detroit NEVER EVER made any engines for light duty vehicles for GM or anyone else. The Olds 5.7L, and GM 6.2L/6.5L engines were NOT made or engineered by "Detroit Diesel", but by GM of which Detroit Diesel WAS a subsidiary.
