Lol. It sounds like your car is old and not in the greatest shape of its life. There seem to be multiple problems here...

1) possibly ignition problems--put in new spark plugs, and check your wires for any bare spots or cracks in the insulation

2) If you can hear air hissing near your engine, you could have a leak somewhere, maybe in the intake manifold somewhere. Use a piece of vac line like a stethoscope to try and find where the leak is coming from.

3) The exhaust should be working lol . Jack up the car and take a look underneath to see if you're missing any nuts on your exhaust bolts or if there are any rusted out parts on your piping.

4) The engine shaking a lot could be a broken or loose motor mount. Check your motor mounts and tighten down the nuts if they are not already tight.

It might be time for a real tune-up by a mechanic. It may run you a few hundred bucks if you have him replace many parts, but if you leave your car like this it will only decline over time anyway.
