i don't know about the codes but to me it sounds like when you give it the gas it stumbles, which to me means not enough fuel is being delivered.

maybe start off by trying some good fuel system cleaner in the tank, if that doesn't do it, start changing and checking each part, fuel filter, injectors, pump maybe.
