I ended up going with the Gibson cat-back "Super-Truck" system. It exits in front of the rear wheels and is pretty loud. This system was easy to install, since it doesn't go over the axle.

Gibson Exhaust for your application. They offer both stainless steel and aluminized systems with a variety of exit locations. I noticed that the exhaust on my 03 was 2 3/4" diameter and no one offers exhaust components in this size. The Gibson system is 3" between the cat and muffler.
