dodge 2001 transmission problems

My wife pulled off the interstate from the time of the off ramp to the get on the other road she pushed the gas and there was nothing there and there was no prior issues. there is a 125,000 miles on the van. now sometimes moves forward and will not move in reverse. Here are the codes found.

Is my tranny toast? Or is there other things i can check?

Try checking all the connections for the TCM, input and output sensors to make sure they are not loose, corroded, oily, etc. When was it last serviced??
