idle issue and noise when in drive

hi all ... just got a 1994 e250 4.9l .. runs great but idles high and fluctuates, from what i have been reading maybe a vacuum leak??? the other issue is when in drive it sounds like a minor exhaust leak. almost like a manifold, but when not in gear very smooth no leak... seems strange..
