There is no adjustment,it is hydraulic.Could be a bad slave cylinder and the transmission does not have to come out,it is on outside

You can try what's suggested by the others but it sounds like the clutch is done.
Time for a new clutch.

You get a kit for the clutch with a new throw out bearing, clutch and pressure plate, bearing for the input shaft which goes in the crankshaft and the line up tool. You change the pressure plate, clutch and throw out bearing. Also inspect the flywheel for hot spots,it would not hurt to get the flywheel resurfaced and any machine shop will do this.Do not get a rebuilt kit, they are a big piece of junk and you will have problems.Make sure you spray the pressure plate down with brake clean very good, has a coating to prevent rusting.

the 5 speed in those trucks do let out at the top. i have noticed it going from other cars then to my truck, it is normal. if the clutch slips then it would be time to replace it.

I have a '93 with the 4 speed. It didn't do that. The transmission had to come out because the clutch was so stiff it kept blowing out the master cylinder. The clutch was almost worn out, but it engaged at the normal height and felt no difference after the clutch was replaced. The stiff clutch was due to a cracked clutch fork. If the transmission needs to come out, I'd replace the fork as well.
