van jerks while driving

I have a 97 dodge caravn that starts jerking while I am driving. also while I am at a stop light seems to miss alot like it's going to shut off. my husband has replaced the spark plugs, added a fuel injection cleaner, replaced the fuel filter. Still jerking. Please advise on what might stop this jerking while driving. thank you,

Do you have a service engine soon light and have you checked for fault codes using either the key dance or a scanner? Does it "jerk" with transaxle in neutral? Without more info I'd change the plug wires and feel/listen for a consistant click at each fuel injector.

Which engine? When did it start?

Sound like you have some trans problems. Tork converter may be lock up. You will need to bring it to a shop.
