EGR 402 Excessive Flow

2007 Turbo Diesel. 96K Miles (apx)
Has had chronic EGR 402 diagnostic codes popping up.
Dealer did a cleaning procedure and charged $90. Code came back right away (within a couple days).
Dealer replaced EGR under warranty. (100K warranty). Couple weeks later code came back.
I picked up a code reader so I could diplay codes and reset them myself. I have reset this each time, and in the last week the EGR 402 Excessive flow code has popped up three times.
Obviously there is a problem or would not be coming back so often. Bad sensor? Dealer didn't really replace it? Bad wire? Loose connection?
It is due for an oil change and a new fuel filter, but I can not figure any way that could be related. (Yes, I plan to take care of the oil change and fuel filter this week). Actually the truck says its not due for an oil change, but I do oil changes every 5K not the million K before the truck says.
