'98 Taurus Tranny Questions

Well my mom has had three tauruses now, transmissions went or are going out on all three.
When she bought her 03 she gave me her 98 for a winter car. I live in minnesota so it's cold 4+ months of the year.
The transmission always slipped when she gave it to me -WHEN IT WAS COLD OUT-. Once the car warmed up and got to average temperature it would act fine. Now it started slipping in warm weather now too. We changed the fluid and gasket(i think?)
The only time it starts slipping is when its cold like I mentioned, if it gets rev'd enough it will finally kick in but I'm sure thats not good for it. If it does get revved past it will catch and go into second and it won't really have any problems that I notice.
My questions are:
Is there any easy fix I may be able to do?
Am I going to end up on the road somewhere waiting for someone to pick me up soon?
Could someone with decent mechanical experience do a tranny swap if I was to get one out of a junkyard somewhere?
