'96 Accord code 16 injectors not firing

I have a '96 Accord that had an engine swap. Replaced the original with the same engine/USDM F22B1. Nothing modified in this car. My mechanic is getting a P0200 code not 16 (CORRECTION) and injectors are not firing. He has traced everything to death. I got him another PCM to see if that was an issue. It was no help. I have searched troubleshooting forums and got:
-check fuel injection relay/re-solder
-swap injectors
-trace wires for ECU or PCM grounds
My mechanical experience is telling me a pinched or disconnected wire, bad ground possibly the motor needs to come back out to track the problem. Some feedback would be much appreciated. Thanks

I'm a little confused...a '96 should be OBDII, which doesn't use the simple two digit trouble codes that OBDI did....how are you getting this code, and what is the production date on the car?

My mistake. You are correct, it is OBD2. It is '96, not sure exact date/car is in a distant location. My guy wasn't specific, just stated it was injector malfunc. code that keeps coming up. Not a specific cylinder. Maybe a P0200? I wish I could provide more info, but I having some communication failure with him. Unfortunatly, I had to move for a new job and could not get this car finished in time. Paying the price now.
