It should last a long time, unless you have problems with your suspension...

A waste of sixty bucks every year if you ask me.
It all depends on your car. I have had my G20 done once, in the four years I have owned it. My civic, once, my wifes mustang, never.
You are the one that determines when you think it should be done. But I am sure there is some graph or chart that says otherwise.

When you notice tire wear or your car pulling to one side.

On a lowerd car you may have to do it more often because ever little bump you hit rattles the suspention more then beening higher. Once every couple years is about right unless you have worn suspention parts, tie rod ends, ball joints, or bushings. ALSO the more bupms you hit the more likely you will have to align it sooner. If you hit a curb or pot holes alot you will be up for 6 months or so.
