I am replacing the starter on a 1991 Accord, and I see starters that are made by Denso and Mitsuba. How do I know which one I need or does it matter?

The answer is the one you were hoping was unnecessary - you go under the vehicle and look at the label on the starter. If you can't find the label, you remove the starter and bring it with you to ensure you get the right one at the parts store.
FYI - not sure if it's the motor or the solenoid on yours, but I just replaced the contacts on my denso starter solenoid on my van over the weekend to the tune of $4.86 and solved my cold starting problems. A remanufactured unit would have cost me ~$120.
