honda accord strange noise

my honda accord has recently been making some kind of weird clutching sound in the front during drive. it gets louder, and car shakes more the further i drive. its worse on concrete roads. took it to the local mechanic, who told me it was a loose centric. he tightened it,4 hours later, noise is bak? help
