System specs: P4 3.0ghz
1.8gb ram
ATi radeon x1650pro AGP 512mb
After playing a game for about 30 min, the graphics go haywire. I have a large fan blowing across the card in addition to the built in card fan. The temp of the card is around 125 F. Do I need more cooling or what?

What do you mean "the graphics go haywire"? What are you using to measure card temp?

All of a sudden I get like large 'planes' of grey/black/white all over the screen and I cant see whats going on. Sometimes they are lined up with certain axis like the cars centerline or axles, etc. Ill try to get a screenshot. I used the Ati catalyst control center to see what the temp was. it reads around 54 degrees C.

Didnt the Card come with diagnostic software?
Did you run the setup tests when you installed the card to set the peramiters that work best with your PC?
Could be one of the settings causing this.
I have the ATI cards as well and I have had problems with certain games.
Either openGL or direct3D was making the screen flicker with a test pattern . I just went in and adjusted the settings until it worked.
Though that is awfull hot !

I got a Zalman GPU cooler and that particular problem went away. Now the car is around 50C-55c at all times. The only other problem is when watching streaming videos online, occasionally they appear only as a few colors like green , yellow, red etc. Just looks like the colors are all wrong.

Depending on the brand, you may be able to get a replacement. I have a feeling it's a faulty card. Some brand have lifetime warranty like EVGA.

Though that is awfull hot !Dunno what you're talking about but 125f isn't hot for a graphics card.. It's actually below average.... My 6600gt oc tips 170F under hard gaming and that's not even hot around 96-100 idle... Most cards run 125f+ @ idle..
BTW, are you measuring temps WHILE playing or playing minimizing it and checking the temps?.. If you do that the card has time to cool down around 30-50 degrees (I know this sounds wrong but its true).
