Car dies on the road

Hello!
I have a Chrysler Vision, 3.5L, '95. While driving down the road the car just dies. When I pull over and try to start it again engine cranks fine but won't start. If I leave the car for an hour or so then it will start again but soon dies again. Error code is "12 - direct battery input to pcm was disconnected within last 50 key turn on"... No other codes!
Any Ideas? THANKS!
