328 Idle problem - worse when cold

Hey all--
A buddy of mine has a '99 328 with about 74K miles and 2.8 L six, automatic.
When cold, the idle varies between about 900 and 200, and it will usually stall on him unless he gives it some gas.
Once warmed, the idle still varies, but only about 100-200 rpm.
This is all in park, and he says that otherwise the car runs fine.
I did a search here but didn't find much; any ideas?

Yes...the vacuum system was clotted up with carbon, and had to be replaced. I think it was a $600 job but covered (for him) under warranty.
