Hesitation Problems

I own a '93 Grand Marquis (24,000 miles). Recently I have been having problems with it hesitating/jerking while driving. The problem usually happens around 40 after settling into a gear and holding the pedal steady. Pretty bad jerking / hesitation problems. Help please. Does anyone know what might be the problem??
