Welcome to AF !
For one the engine is metal, and it rusts. The radiator is brass or aluminum.
After several years with out proper maintenace of the cooling system it will develope rust in there and it will build up in the radiators fins and could eventually clog it up.
By keeping fresh(and the proper amount) antifreeze in your system it will protect it from rust.
Run a couple bottles of radiator flush(prestones good) through the system as it says on the bottle(s).
I generally leave it in 10-15minutes longer than what the directions say.
It will take several times of filling and draining to clear out all the cleaner and rust.
Good Luck

Fans are set to kick on at a certain point based on specs. If it is overheating it could be due to needing a flush. As stated above if not cleaned the radiator fins could get clogged thus not as much cooling effect.

Yes it will cause it to overheat.
Would not hurt to take off the resivour and clean it out too.
Also Other areas such as inside the engine can also build up rust and plug water jackets in the heads, thus causing overheating.
If you hav the $$ get a gallon of distilled water instead of rust prone tap water, along with your anitfreeze
