what does the cam gear do adjust ?

It adjusts when the intake and exhaust valves open and close. You only need it when you either mill the head (which retards timing) or if you're using an aftermarket cam (most likely not with a stock replacement or a stage 1 cam though). But a lot of people buy them just for looks and never end up adjusting them at all.
