O2 Sensor Replacement

1999 540i has symptoms indicating an O2 sensor may need replacing. This model has four (4) O2 sensors. Do all four have to be replaced, or how do you determine WHICH one needs replacing? And, why do they go bad in the first place? Thanks!:grinyes:
