How many miles? Has the tranny fluid ever been changed? Sometimes those problems are caused by sticking or failing shift solenoids and the fluid change solves the problem. As a last shot before dropping the valve body you might try some Lucas TransFix. I'm not much of a believer in additives to alleviate a mechanical problem, but they do have their place.
FWIW
YMMV

This should be scanned ( only a trans capable scanner will work) for possible problems with the shift solenoid or the pcs solenoid, as Df bonnet says, the adaptive shift percentages can point to the problem

It is not a matter of codes, it is a matter of analyzing live data from the trans info.
