Need a 03 - 05 inside mirror wiring

I have a mirror out of a 03 Suburban but I am having a hard time finding the wire scimatic. Can any one help

Here are the basics. The connector is numbered (look closely).

5- outside mirror +
6- temperature probe
7- temperature probe
8- ground
11- OnStar keypad out
12-OnStar keypad power
13- ignition
16 - outside mirror -
